For more information, visit https://www.albertahealthservices.ca/cc/Page15339.aspx. We are looking for a Social Worker II to join the Transition Services and Seniors Health Portfolio in Central Zone. The position will provide social work support to the Drayton Valley Hospital and Home Care. The Social Worker II is responsible for the provision of social work services to clients and families including psychosocial assessment, crisis intervention, education, advocacy, resource finding, consultation, supportive/adjustment counselling and making appropriate referrals to members of the multidisciplinary team. This position facilitates complex discharges from Acute Care to the Home and works collaboratively with Acute Care Transition Coordinators and multidisciplinary teams. The position supports clients and their families through their care journey in hospital, their homes, and facilities. The successful candidate will work in multiple program areas including Home Care, Supportive Living, Facility Living, Acute Care and Palliative Care. In addition, Social Worker II work within a Competency Framework that includes personal and professional accountability, systems thinking, creative thinking, communicating, and building relationships.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
