Interested candidates MUST submit a cover letter & resume, outlining their interest in working with Elbow River Healing Lodge (ERHL), their knowledge of health inequities within an Indigenous population, and their relevant skills & experience. ERHL is an Indigenous focused primary health care clinic which offers holistic, wraparound healthcare to First Nations, Inuit, and Métis individuals and families. The goal of ERHL is to provide access to care that is safe and respectful of cultural beliefs. ERHL is made up of an inter-disciplinary team providing medical support from physicians, nurses, allied health professionals, and specialists. Connection to traditional FNMI healing and cultural services is supported by the Traditional Wellness Coordinator. Outreach social work and nursing staff help clients with crisis support, resource navigation, case management, and advocacy. The vision of ERHL is to have healthy Indigenous communities that are connected and empowered to access healthcare. As the Social Worker II (SW II) you will have the opportunity to make a difference in the lives of Indigenous patients and families. The SWII performs a wide range of social work duties including crisis counselling, and support services to promote the health and wellbeing of clients experiencing social, financial and/or health challenges. A key component to this position will be liaising with community organizations and Indigenous supports within Calgary and surrounding area, assisting clients with resource navigation, and advocacy. The SWII advocates for culturally competent health care and community services. The SWII must be able to work independently and in an outreach capacity, and also to work effectively as part of an interdisciplinary team. The SWII will commit to inter-professional collaboration to enhance patient health outcomes.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Minimum 3 years experience working with Indigenous populations. Experience in advocating for clients by coordinating the efforts of all internal and external parties to meet client and family psychosocial and instrumental needs such as housing, food, transportation, income support and financial management. Knowledge of resources specific to Indigenous populations is required. Minimum 3 years recent community or primary care experience. Experience working with complex high-risk clients, addictions, & mental health with potential exposure to verbal aggression. Understanding & training with Anti-oppressive practice, Stages of Change, & Trauma Informed Care. Demonstrated attention to detail & accuracy. Strong communication skills with ability to relay patient information to other healthcare providers in a timely, concise, accurate manner in order to facilitate treatment decisions. Ability to balance numerous demands & conflicting priorities. Experience providing crisis counseling, mediation, & conflict resolution strategies. Established relationships with Indigenous serving agencies, Nations, & supports in Calgary & surrounding area. Experience with an EMR system (Connect Care) and demonstrated aptitude with computers and technology. Extensive knowledge of the benefits and financial supports available to Indigenous clients is necessary.

Experience supporting clients with residential school settlement applications. Experience with AISH & other financial support applications. Experience with the Child & Family Services system. Experience with individual & group interventions.
