The Social Worker II Assisted Living Alberta plays an essential role in supporting patient transitions and coordinating care across Acute Care, Home and Community Care, and Continuing Care Home settings. This position delivers comprehensive clinical social work services with a strong emphasis on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. The ideal candidate demonstrates strong clinical judgment, exceptional communication skills, and the ability to work both independently and within a collaborative healthcare environment. Success in this role requires adaptability, critical thinking, and a commitment to delivering patient centered care in a dynamic and fast paced setting. This position primarily supports the Emergency Department at the Northern Lights Regional Health Centre, with occasional coverage in other care settings and units within the Fort McMurray area as operational needs arise. The role requires strong knowledge of community resources, housing options, Indigenous communities and supports, and provincial and federal programs that assist vulnerable populations in accessing stable housing and essential services. The successful candidate must be comfortable working with individuals experiencing vulnerabilities such as addiction, food insecurity, and housing instability, and be able to provide trauma informed, culturally sensitive support within a fast paced environment. In addition to a competitive rate of pay, Assisted Living Alberta currently offers a Fort McMurray Allowance of up to $12, 480 annually. This allowance is non pensionable and paid hourly for all hours worked at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Certification in Basic Life Support - Healthcare Provider.

Connect Care training. Non Violent Crisis Intervention certification. Designation as a Capacity Assessor. A minimum of one year of experience in Acute Care, Continuing Care, and/or Home Care. Enhanced knowledge of community based supports and services. Experience and knowledge related to AHS/ALA system navigation, care placement processes, transitions in care, and available financial supports.
