Description
At the Silver Willow Lodge in Nanton, we provide personal and professional care services in Type B Continuing Care for clients with needs that fall within the psychosocial, physiological, health related behavior, and environmental domains. Under the ISFL umbrella, we are recruiting a Registered Nurse in the role of a Case Manager to coordinate the placement, assessments of on-going referral needs, care planning (both complex and annual), and delivery of professional and personal care services to our clients. NOTE: This position is located in Nanton, about 90Km outside of Calgary.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Leadership skills, the ability to build relationships and champion change. Excellent organizational and time management skills - be able to efficiently organize many details in a time efficient manner. Minimum of 5 years current nursing experience. Case Management experience. The successful candidate will work independently in a supportive living setting. Physically able to perform all assigned tasks. Able to sit for long periods, occasionally walk long distances and to be able to carry 25 lbs. Ability to be on the phone or work on the computer for long periods of time. Computer proficiency - Excel, Word, MS Office and Connect Care. Strong computer and technology skills (or ability to quickly learn and adapt) is required due to electronic documentation and care planning requirements. Strong negotiation and mitigation skills; ability to remain professional and composed during times of adversity. Excellent verbal and written communication skills required. Ability to work independently and with minimal supervision required. Experience with comprehensive physical assessments. Ability to problem solve and communicate abnormal findings within appropriate time frames based on a risk assessment. Geriatric and dementia experience/certification. InterRAI experience.
Baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Recent experience with the Integrated Supportive and Facility Living team. Palliative experience is an asset.
