This exciting opportunity as Registered Nurse, Designated Float covers planned and unplanned staff absences. Integrated Home Care uses a strength based Client Centered Case Management practice framework. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with short term and long-term clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills. This role requires flexibility, adaptability and rural/remote travelling. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. The Rural South team is comprised of Diamond Valley, Okotoks, & Vulcan, the farthest site is 130 km south of the Calgary city Limits.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. The candidate must have at least 5 years of nursing experience with a minimum of 3 years of recent Home Care nursing experience. Level 2 McMaster AHS Case Management education. A valid Driver’s License. Ability to lift 30 lbs. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. Experience providing case management, client teaching and professional nursing interventions including but not limited to Home Parenteral Therapy (HPT), wound care, ostomy care. Experience supporting vulnerable clients those requiring palliative, mental health and/or addictions support.

The ideal candidate will have: a bachelor’s degree in nursing; Experience working with acute clients and/or long-term clients with a focus on chronic disease management. Recent work experience in a community-based setting. Home Care experience preferred.
