Description
If you like small town living, this position in Drayton Valley might be for you! Drayton Valley is located in the Central Zone, just 133 km southwest of Edmonton and 172 km northwest of Red Deer. The town has a population of over 7,000. In and around the area are a library, movie theatre, bowling alley, camping areas, 6 golf courses, and a ski hill. The Omniplex encompasses three multi-purpose recreation facilities, all under one roof. Conveniently located in the heart of Lions West Valley Park, the Omniplex is surrounded by five baseball diamonds, a soccer field, the rodeo grounds, a secluded day-use park and a 40-site full-service RV Park. The Drayton Valley Home Care program serves a large area of clients including the community of Drayton Valley and surrounding areas. We work closely with community groups, other health care services in the community and as part of a multidisciplinary team. Registered Nurse - Case Manager is responsible for directing the care of clients under the home care program. The Case Manager must have strong interpersonal communication and interdisciplinary team player skills, excellent critical thinking, creativity, resourcefulness, time management, and organization abilities. The Case Manager responsibilities include assessment of client needs, establishing goals for client care delivery, planning, delivering, and coordination of care needs, resources, and services within professional practice standards and home care program guidelines, communication and coordination of client care with the interdisciplinary team; and evaluation of outcomes.
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Ability to be on call as per operational needs and report to the site/client’s home for on call/call back duties within 45 minutes of call. Ability to meet the physical demands of the job.
Training in case management principles is an asset. Knowledge of the continuing care sector and various levels of care is an asset. Knowledge of Connect Care and interRAI HC is preferred. Experience in wound care with excellent knowledge of wound care principles and wound care product use is an asset.
