Skypointe II Complex Mental Health Rehabilitation Program is an Addiction and Mental Health program located in the community of Skyview in Calgary providing 24/7 care to 32 individuals. Team members collaborate with clients, family, care providers, other programs and agencies to provide quality, client-driven, culturally appropriate, and integrated services. Utilizing principles of Patient Family Centered Care, Recovery, Trauma Informed and Psychosocial models of care, the Outreach Workers (ORW) will work collaboratively with the interdisciplinary team (Recreation Therapist, Occupational Therapist, Nursing, Behavior Specialist, Social Workers, Admin, Protective Services, and Physicians) as well as partner agency staff to provide care to clients both on site and in the community. The facility offers craft areas, library, group facilitation, quiet rooms, and a fitness area to support the rehabilitation of the clients. As a transitional program, ORW provides individualized and holistic support and interventions in collaboration with the client, staff team, family/guardians/other supports, and other involved professionals as required. The ORW's play a pivotal role to enable access to available on-site and community amenities. This includes practical intervention to assist with ADLs, hygiene, clothing, financial assistance, money management, maintain living space, transit training etc. The ORW will work with clients to develop time management, problem-solving, prioritizing and organizational skills, where repetition has been an identified need. The ORW may be required to deal with clients who are unpredictable or aggressive and must be able to adequately de-escalate and respond appropriately. As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

Completion of diploma from a post-secondary program in a related field.

Minimum 6 months experience in the last two years in an acute care or community Addiction and Mental Health setting. A willingness to transport clients is required. Experience working with multidisciplinary teams and community agencies. Knowledge of relevant community resources and legislation. Must be able to lift 20lb, climb 3 flights of stairs and ambulate quickly. This position has transitioned to Recovery Alberta effective September 1, 2024. 2 years’ experience in acute care or community addiction and mental health setting preferred. Awareness of the DSM-5 with respect to mental health diagnosis, symptomatology and interventions.
