Description
Work independently with your own panel of patients of all ages, supported by a strong, collaborative team. You will be part of a group that includes other Nurse Practitioners, Physicians, Nurses, Administrative Support Workers, Psychologists, Social Workers, Dietitians, a Pharmacist, Data Analyst and both a Physical and Occupational Therapist. East Calgary Family Care Clinic (ECFCC) provides comprehensive primary care for patients with complex needs. This includes a mix of chronic conditions, mental health concerns, and social or economic challenges that affect health. Nurse Practitioners at ECFCC work independently with their own panel of patients and serve as the most responsible provider for their care. You will work as part of a collaborative, interdisciplinary team, providing patient- and family-centered care using a nonjudgmental, trauma-informed approach that supports healthy behaviors. This role is a great fit for someone who wants to work to their full scope of practice in a team-based setting. ECFCC is open 7 days a week with extended hours. This role includes occasional evenings, weekends, and statutory holidays. In the 8-week rotation, there are typically 4 evening shifts (12:45 PM to 9:00 PM) and 2 weekend shifts (8:45 AM to 5:00 PM). As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
You bring flexibility and adaptability to work in a complex, fast-changing environment. You apply principles of primary health care and trauma-informed care in your practice. Strong organizational skills, effective communication, and the ability to work well in a team are essential. You also have solid computer skills and experience using an Electronic Medical Record (EMR) system.
Experience as an NP and RN in chronic disease management, urgent or episodic care, mental health, and substance use disorders is an asset. Familiarity with Connect Care is also preferred.
