The NP in the Heart Function Clinic (HFC) is a self-directed, advanced practice nurse who functions in a collaborative practice model within a diverse interdisciplinary team. The HFC NP demonstrates comprehensive knowledge in evidenced informed heart failure diagnosis and management supported by foundational knowledge, skills and experience in general cardiology. This advanced skill set supports the assessment, diagnosis (including ordering and interpretation of tests), intervention (including prescription of medications), management, referral and evaluation of heart failure patients across the continuum of health care services. The HFC NP will primarily focus on direct patient care as a central member of the team in the heart failure specialty clinic. Additionally, the NP engages in critical appraisal of the literature regarding heart failure and related cardiac conditions and actively participates in education and the development of clinical practice tools to support the interdisciplinary team in maintaining evidence informed practice. The NP participates in program development and evaluation, continuous quality improvement and select inter-professional committees as required. As an educator, the NP will engage in ongoing surveillance of the knowledge needs of health care professionals, patients, families and communities dealing with heart failure and contributes to the provision of education to meet those needs.

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Registration with CRNA on the Nurse Practitioner Registrar including Provisional Graduate Nurse Practitioner licensure; Successful completion of an approved Nurse Practitioner program and a minimum of master’s degree academic preparation; Successful completion of a Baccalaureate of Nursing degree and minimum required hours of RN practice. Minimum of 2 years recent experience as an adult NP, preferably in a relevant clinical setting. Computer literacy skills mandatory (Including but not limited to keyboarding, Microsoft Word, Power Point, Outlook, Excel). Excellent written and verbal communication skills.

Minimum of 5 years recent cardiac nursing experience in the Critical Cardiac Unit/Coronary Care Unit or Intensive Care Unit and/or community-based specialty heart failure management clinic. Experience with chronic disease management and vascular risk reduction in the community setting. Experience in a wide range of cardiac assessments, treatments, therapeutic interventions, diagnostic procedures, follow up care and health promotion.
