Description
Located within the Rocky Mountain House Health Centre, you will work with a dynamic, supportive team and will have the opportunity to make a difference in the lives of people in your community. Rocky Mountain House is located 77 km west of Red Deer in west - central Alberta on the North Saskatchewan River. This multicultural community has that small town feel with big city amenities. As a Licensed Practical Nurse, you will report to the Home Care Manager and will be responsible to assess, plan, monitor and evaluate clients in consultation with the Registered Nurse Case Manager. You will be responsible for coordinating and providing quality care to clients in the community and will provide direct care in partnership with clients and their families in their own residence or an alternate.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.
Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
A proven track record of reliable attendance, dependability, and professional conduct is essential and will be verified through employment references.
Skilled in Home Parenteral Therapy, wound care, lower limb assessments, skin and ostomy care, palliative support, and client teaching. Knowledge of Continuing Care Health Service Standards. Minimum two years of recent Home Care experience or relevant community healthcare experience with strong clinical and care coordination skills.
