Description
The Licensed Practical Nurse (LPN) reports directly to the Diabetes and Endocrinology Clinic Manager and plays a vital role in an interdisciplinary team dedicated to delivering high-quality, patient-centered care in an outpatient setting. The LPN supports a holistic, integrated approach to healthcare and health promotion by applying the nursing process, critical thinking, problem-solving, and decision-making skills. Key responsibilities include conducting initial clinical assessments-both in-person and via telephone-triaging patients, and coordinating care for individuals enrolled in both the endocrinology and diabetes programs. As part of the diabetes care team, the LPN will be responsible for preparing and reviewing insulin pump data in advance of clinic visits. This role also serves as a liaison between patients, the interdisciplinary healthcare team, and external healthcare providers to ensure effective communication and continuity of care.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.
Completion of an accredited practical nurse education program. Active, or eligible for, registration and a practice permit as a Licensed Practical Nurse with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Connect Care experience and proficiency with reviewing electronic lab results required. Current experience within the last 3 years in an Ambulatory Care setting required. Current registration with Professional Body: CLPNA. Current BLS-HCP with the physical ability to perform CPR on adult patients. An intermediate level of physical activity is required to fulfill job requirements including but not limited to reaching above shoulders frequently, looking up frequently, prolonged periods of keyboard computer work and the ability to assist patients with mobility limitations is required (ie: transfers, bariatric clients, bending, stooping, twisting, walking distances). Must be able to lift and carry 10-15kg regularly. Must be physically able to lift 50 pounds. Must have excellent communication and documentation skills, and the ability to participate as an interdisciplinary team member. The candidate needs to be able to work independently and needs to have high organizational and prioritizing skills. A working knowledge Connect Care, Netcare, Outlook and Word as well as other applications is required. Responsible for continuing competence through ongoing professional development including participation in education programs, research, and continuous improvement activities.
Knowledge of insulin delivery systems.
