Description
If you enjoy a role in providing services to clients in a community-based setting, then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. Working as a Case Manager, the LPN is accountable for assigned caseloads with the primary responsibility to assess client needs, determine service needs, negotiate service options, care planning, and coordination of transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to the client need. There is also an expectation to be a mobile worker. This role requires travel up to 100 km/day caring for clients in their homes or in Rural community. You will be responsible to operate a laptop computer and mobile phone. Standing, walking, lifting, pushing, pulling, stooping and carrying equipment is required throughout the shift.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.
Completion of an accredited practical nurse education program. Active, or eligible for, registration and a practice permit as a Licensed Practical Nurse with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Minimum of two years current nursing experience in an Acute Care setting. Recent experience working with complex wounds, surgical drains, & IV (HPTP) therapy. Knowledge, skill, ability and willingness to work to full scope of practice. Commitment to continuous learning relevant to LPN practice. Ability to utilize critical thinking skills in the practice of conducting initial and ongoing nursing assessments, care planning and service delivery. Ability to utilize critical inquiry to assess, plan and evaluate the effectiveness of interventions on client outcomes. Excellent verbal and written communication skills Basic use of technology and competency in computer, clinical electronic device use, and electronic documentation.
Completion of a post-basic Medication Administration Course for LPNs. Recent work experience in a community-based setting, Home Care experience preferred. Experience working with complex chronic disease(s) and Palliative clients. Experience in Case management practice and completion of the McMaster AHS Case Management education. Advanced wound care certificate.
