Description
Milk River Home Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our community focused team. In this role, you will support clients to safely remain in their homes by delivering high quality, patient and family centered care. You will play an essential role within an interprofessional health care team, using your nursing knowledge, critical thinking, and clinical judgment to promote independence, dignity, and optimal health outcomes for clients across the lifespan. This position also provides clinical support to the Type B HCA facility, Prairie Rose Lodge, ensuring residents receive consistent, person-centered care. You will work closely with Health Care Aides, offering assessment, guidance, and leadership to support safe, effective care delivery within the lodge environment. Guided by your education, experience, and demonstrated competence, you will practice in alignment with all relevant legislation, standards of practice, and organizational policies and procedures. This role offers a balance of independent practice and collaborative teamwork, requiring strong assessment, communication, and problem-solving skills as you support clients with varying levels of complexity. As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.
Completion of an accredited practical nurse education program. Active, or eligible for, registration and a practice permit as a Licensed Practical Nurse with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Recent clinical experience functioning at full scope of practice (Physical Assessment, Wound care/training, Medications, IV therapy). Time management and organizational skills. Effective oral and written communication. Experience with Connect Care. Ability to prioritize and work collaboratively with a multi-disciplinary team. Ability to understand and carry out nursing activities in accordance with the Continuing Care Health Service Standards. Knowledge of Wellness, Independence Model of Care and Patient/Family Centered Care. Knowledge of community resources. Ability to work independently and as a member of a team in a fast paced; ever changing environment. Ability to work with people in a health care environment with professionalism and respect. Negotiating skills with clients, caregivers and families. Ability to perform all assigned tasks and direct clinical care including but not limited to: constant adjustment of position (sit/stand) throughout the shift, physical exertion, including bending/walking/sitting or standing for long periods of time, keyboarding/mousing and documentations for prolonged periods of time. Experience caring for palliative clients and families. Recent clinical experience in psychiatric and geriatric nursing.
Home care experience within the last 2 years.
