Description
This position is currently in Grande Prairie within a supportive office environment. You will be working in conjunction with Case Managers as well as work with HCAs that are supervised by the Personal Support Coordinators. Additionally, you will work with allied health, community paramedics, EMS, lodge partners, pharmacies and physicians. This type of nursing requires someone with a strong skillset in assessment and critical thinking. You will be working with clients from birth to end of life spectrum. You will be required to work collaboratively within a multi-disciplinary team as well as with the clients and their support networks. You will provide care to clients needing frontline nursing support and assist the HCAs trouble-shoot issues with the medications assistance program (MAP) and work with the Case Managers to identify risks to clients and possible interventions. You will be going to lodges and community clients’ homes, often independently. You will bring to the position your health care knowledge, strong nursing & communication skills, positive approach and willingness to work as part of a team as well as an ability to work independently. You will actively participate in quality improvement initiatives. You will participate in assigned committees/working groups/meetings. You must have available, for work purposes at all times, a reliable vehicle with business insurance. You must be comfortable driving in any weather/road conditions. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
1-2 year’s experience in Home Care. 3-5 years LPN experience dealing with all entire age spectrum including seniors. Currently registered with CLPNA as an LPN in good standing. Be able to work independently as well as part of a collaborative team. Strong communication skills.
