Description
The Behaviour Change Consultant provides content expertise regarding behaviour change and self-management skills and resources to support client service delivery, program development and evaluation, provider education and strategic planning. The position also provides direct service delivery for clients and providers. This position will support to Edmonton and area Primary Care Alberta programs and services, including Primary Care Networks and other partner organizations in the communities. This position reports to the Manager, Chronic Disease Management. This position is part of Primary Care Alberta. As a Health Educator II, you will participate in the development, provision, implementation, enhancement, and evaluation of new and existing programs and services. You will collect data and discuss health concerns and strategies with program consultants, frontline staff, physicians, managers and community primary care providers. In this leadership role you will teach, mentor, and coach staff as well as provide health and wellness education to the public, patients, and providers.
Completion of bachelor's degree in a health related field.
Minimum of three years' experience in community health based adult education and working with persons with chronic illnesses. Experience educating health professionals and peers. Experience working with Vulnerable populations.
Master’s level education or equivalent experience desired. Minimum of three years' experience and leadership in program planning, coordination, delivery and evaluation desired. Evidence of ongoing professional development in the fields of behavior change, HealthChange ® Methodology, self-management and adult education. Experience with primary care, specialty care and work experience in varied community settings, including working with marginalized populations, is an asset.
