Description
Assisted Living Alberta is dedicated to enhancing care in the community, helping people/families be healthy, well and independent in their homes and communities. Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Specialty Programs. The Specialty Programs portfolio includes Children’s Home Care (CHC). CHC is a multidisciplinary team with RN, SW, RT, PT, OT, SLP and TAs collaborating and delivering services to children from birth - 18 years old with a wide variety of medical and/or developmental needs/conditions, including palliative care. The CHC program serves the Edmonton Zone, and frequently collaborates with suburban/rural Home Living partners to provide quality care. The Manager is responsible for overseeing the appropriate delivery of Home Care services to eligible pediatric recipients and their families within the Edmonton Zone. The incumbent will provide strong leadership and mentoring to front-line staff, manage resource allocation, and participate in/support change management. Strong communication skills are essential to promote and maintain internal and external, as well as cross-ministerial partnerships essential in the delivery of pediatric focused services.
Resource Management: Monitors & manages the day-to-day operations of professional staff, the dynamic workload challenges, & its effect on budget. Service Delivery: Fosters client centered care, recognizing unique client situations & complexity of care delivery in the community. Due to the complexity of the children/families managed in community, the manager is often directly engaged in supporting the staff managing difficult situations & actively participating in planning for acute care discharges and other transition points throughout a child’s/young adult’s life. Monitors effectiveness of contracted services provided by home support agencies. Collaborates with staff to resolve complex client/family issues/risk issues. Reports issues to the Program Manager that impact, or have the potential to impact, the network/program area or the zone. Provides feedback on operational decisions within the assigned area of responsibility based on critical review of clinical information, workload data and fiscal resources. Coordination & Communication: Engages with internal and external partnerships to promote an understanding of available HC resources and Home Living operational capacity to meet client assessed needs. Negotiation and conflict management are key skills used to resolve situations with agencies, partners, & families in a solution-focused manner. Staff Development: Fosters an atmosphere conducive to encouraging & meeting staff development needs & student learning experiences. Attendance & Performance Management: Monitors & documents staff attendance & performance. Provides immediate & constructive feedback while working with the staff to facilitate ongoing quality performance. Recruitment & Orientation: Participates in the recruitment process, from identifying the need to facilitating/providing orientation to newly hired staff.
Bachelor’s degree in health related field required. An equivalent combination of experience and education will be considered. A minimum of 5 years of clinical practice or health related experience - community care preferred.
Annual membership in good standing with an accredited professional association is required and must be maintained throughout the duration of employment.
Master’s degree and previous management experience in a health related field is preferred. Computer literacy in Microsoft Outlook, Word, ESP, e-People and electronic meditech/connect care documentation is an asset. Knowledge of the principles of adult learning is an asset. McMaster Case Management training and understanding of the AHS Continuing Care Case Management Competency Framework.
