Nurse Team Lead - Downtown Central FACT

The Downtown Central Flexible Assertive Team (FACT) is looking for a full-time, temporary (until March 19, 2027) Nurse Team lead.  Reporting to the Manager of the Downtown Central Clinic, and working collaboratively with the clinical care leadership group, the Team Lead supports the Manager to anticipate needs of the outpatient clinic over time and collaborates with the clinic’s interprofessional team to provide effective coordination of client/patient-centered care, consistent with CAMH’s vision and strategic directions. The Team Lead will assume responsibility for the communication, liaison and coordination of client/patient admissions, discharges, transfers, and client assignments.  The successful candidate will facilitate service delivery, ensuring a high standard of quality and safety. The Team Lead will ensure that the client/patient assignment is clear and reflects the principles of accountability for care, also ensuring that client/patient assignments are accurate and properly recorded as per established clinic’s processes and CAMH standards. The Team Lead will coordinate staff schedules to ensure appropriate coverage of the clinic and lead daily/weekly team and treatment planning meetings. They will also assume a small caseload of clients.
