Position Summary:
Reporting to the Mental Health Program Manager, you will be a team member of the Mental Health Program working with people with severe and persistent mental illness. As a member of an interprofessional team, you will participate in the assessment and treatment process to proactively engage clients in their recovery goals.

Duties:
You will be working with a client caseload, providing case management and outreach support, and be responsible for conducting psychosocial-based assessments, providing financial and social supports/assistance, and counselling to clients, family, and caregivers from diverse backgrounds. You will assume a role in the client’s psychosocial activities - particularly in addressing the social determinants of health. You will have sound knowledge of severe and persistent mental illness as well as demonstrated familiarity with best practices in the assessment and treatment of these illnesses, including concurrent disorders, eating disorders and recovery principles. Other duties will include formulating treatment plans, completing standardized measurement-based assessments, monitoring the client’s response to therapeutic interventions and communicating with the client’s social and community networks. Knowledge of relevant community resources, performing outreach and working with families/significant others is required. Using your excellent interpersonal skills, you will network with other referral sources to coordinate a smooth flow of clients across the program's various services. You will liaise with community providers and participate in educating clients and their families, as well as mental health providers, both within and outside UHN. Complete applications for Alternate Level of Care (ALC) facilities, completing RMR, follow home first policy, establishing contact with members of the interprofessional team and community agencies to obtain, provide or exchange information relating to patients' psychosocial functioning and health care in the past, present and future. Candidates must be capable of providing supervision to Master's of Social Work students, collaborating with social work faculties on special projects related to the development of the social work field, contributing to the development of best practices unique to the social work field, and teaching other professionals.
