About the Integrated Care Program

The Integrated Care (IC) Program at the University Health Network (UHN) is changing the way healthcare is delivered and experienced in Ontario and beyond. With a patient and caregiver-first approach, we’re breaking down the barriers and burdens of navigating the healthcare system, creating more seamless and personalized care experiences. By blending hospital and community care, the IC Program ensures patients receive the right care, at the right time, and in the right place.

Our innovative model of care supports thousands of patients annually across multiple pathways, including surgery, medicine, transplant, and emergency department care. By delivering advanced care at home, reducing hospital stays, and preventing unnecessary readmissions, the IC Program addresses critical challenges like surgical backlogs and hallway healthcare, while paving the way for modernized, scalable, and sustainable home and community care.

We’re now embarking on an exciting new chapter: building and refining our digital systems to unite UHN with community care providers. This initiative will streamline care coordination, enhance data insights, and empower patients and caregivers with virtual tools - all while driving meaningful population health outcomes.

Reports To: Senior Program Manager, Data Strategy

Status: Temporary Full-Time (4-month contract with opportunity to extend)

Hours: 37.5 hours per week

Salary: $35.35 - $44.19 per hour (To commensurate with experience and consistent with UHN compensation policy)

The Credentialed Trainer (CT) plays a key role in supporting clinicians and staff by delivering instructor-led training on UHN’s clinical systems. This role involves collaboration with the Principal Trainer, application analysts, and subject matter experts to maintain and implement course content and supporting documentation.
