  • Summary of Duties:

    The primary responsibilities of the Team Leader include coordinating the activities necessary for the effective and safe daily operation of K1W, a complex community unit. Serving as an effective role model, the Team Leader will apply the knowledge of change theory to facilitate the most efficient utilization of staff, equipment, and the flow of patients. The Team Leader will engage in weekly interdisciplinary rounds, fostering collaboration and communication to ensure comprehensive patient care and effective coordination of services. The Team Leader will support the admissions and discharge process, including facilitating LHIN referrals when necessary. Coordinate weekly wound huddles and play a pivotal role in the Infor/Grasp interrater processes, ensuring accuracy and consistency in documentation. The Team Leader will liaise with the unit staff to guarantee adequate supplies are readily available for patient care needs. The Team Leader will act as a resource, coach and role model to empower staff and promote strong clinical decision-making and problem solving. The Team Leader will actively participate in program quality improvement initiatives, including monitoring key indicators.

     

    Qualifications/ Skills:

    • Excellent attendance on record
    • Current registration with the Ontario College of Nurses
    • BScN and acute care experience preferred
    • Developing Nursing Leadership Program (DNLP) preferred
    • Previous nursing leadership role is an asset
    • Prior experience with dialysis, palliative, oncology, and complex wound patients preferred
    • Excellent interpersonal, communication and collaborative skills, including the ability to communicate effectively with patients, families, nursing staff, physicians, multidisciplinary team members, and other departments
    • Evidence of ability to work as a strong member of the interprofessional team
    • Demonstrates a well-rounded knowledge of unit day-to-day activities, equipment, organization policies, procedures, best practices and protocols
    • Excellent coaching and conflict resolution skills required
    • Computer literacy required, including Microsoft Outlook, Excel and Word, Point Click Care (PCC), Bed Management System, RM&R and Care Visibility
    • Ability to set priorities, organize patients’ assignments and delegate to other for the overall benefit of the unit
    • Knowledge of and ability to effectively use organizational resources
    • Evidence of commitment to Quality Improvement and Patient Safety
    • Up to date on all mandatory corporate certifications and Course (CPR, WHMIS etc.)
    • Evidence of involvement in unit-based project
