REPOST - Speech Language Pathologist - Restorative Transition Unit - Temporary Part-time - 2025-14978

Summary of Duties:

The Speech-Language Pathologist is responsible for carrying out assessments, treatment and for ongoing evaluation in a busy inpatient transitional care unit.  The Speech-Language Pathologist will provide swallowing and communication services in a consultative model for a diverse patient population, that may include burns, neurological rehab, stroke, oncology, amputee, multiple trauma, and orthopedic populations.

Key Responsibilities:
1.    Assess swallowing and communication to inform customized intervention plans to achieve rehabilitation goals
2.    Implement and monitor swallowing and communication interventions in a timely manner
3.    Document therapeutic progresses in the patient's medical record, workload measurement in accordance with facility and professional standards
4.    Discuss treatment progress with members of the therapeutic team.
5.    Educate patient, family and caregivers to reinforce treatment programs and provide counselling related to communication and/or swallowing difficulties and strategies
6.    Participate in team-building, weekly team rounds, and family meetings as a member of the interprofessional team.
7.    Manage complex discharges within a transitional care program, including education to patients and caregivers, and referring to community programs and services as needed
8.    Maintain and upgrade skills and knowledge through continuing education and seek opportunities for knowledge dissemination
9.    Participate in clinical and interprofessional education activities in support of the Graduate Department of Speech- Language Pathology at the University of Toronto
10.    Participate in SLP Professional Council, Program and hospital-wide meetings, committees and planning as required 
11.    Other reasonable duties as assigned.
12.    Be aware of conditions and measures that may affect the safety of employees, patients, Sisters, students, volunteers, physicians and visitors including but not limited to:  compliance with the safety standards of the Hospital and applicable legislation.
13.    Conduct their work in a safe manner; prevention, identification and reporting of safety issues and problems; taking corrective action, where possible; attendance at safety meetings / training / updates in accordance with the Hospital’s standards.

Qualifications/Skills

14.    Successful completion of a Master’s degree from an accredited University.
15.    Current registration in good standing with CASLPO. 
16.    Minimum of two years experience required in a similar setting (rehab population) preferred.
17.    Knowledge and experience with the adult acquired communication and dysphagia management are assets.  
18.    Experience with videofluoroscopic swallow examinations an asset. 
19.    Demonstrated professional judgment and interpersonal skills in dealing with patients, their families, other health care team members and the community are equally essential skills.
20.    Demonstrated organizational, prioritization, problem-solving and case management skills.
21.    Demonstrated flexibility and ability to work as a team player.
22.    Demonstrated interpretive and problem solving skills.
23.    Proven excellent interpersonal, verbal and written communication skills.
24.    French speaking at an intermediate level is preferred.  
25.    Evidence of good attendance, a satisfactory performance history and a discipline-free record required.
