Summary of Duties:

Sunnybrook Health Sciences Centre has an exciting opportunity for a Physiotherapist to provide Outpatient Care as a member of the interprofessional team in the new Peter Cipriano Cente for Seniors Health. This position supports seniors living in the community who are attending the Geriatric Day Program for up to 8 weeks of rehabilitation, or seniors who need a comprehensive geriatric assessment through the Geriatric Medicine and Specialized Clinics or Geriatric Outreach Services. Working across the Cipriano Centre, physiotherapy resources will be prioritized by senior’s presenting needs and the aim of the setting.

 

Responsibilities include:

  • Practice in accordance with regulatory body Standards of Practice.
  • Support Sunnybrook’s Outpatient Care strategy to achieve integrated and sustainable models of care, working across the clinic, day hospital and outreach team.
  • Assess, plan, implement, direct, and monitor patient’s physiotherapy programs and will work closely with Physiotherapy Assistant when implementing programs.
  • Develop and implement both individual treatment plans and group-based therapy plans
  • Evaluation, treatment and education of conditions commonly seen in the geriatric population
  • Complete Comprehensive Geriatric Assessments (CGA)
  • Collaborate with clients, families and caregivers to promote seniors living in their community
  • Act as a Physiotherapy expert resource for staff, students, and other healthcare providers
  • Participate in team meetings, rounds, teaching, and family conferences to review client care.
  • Maintain and upgrade knowledge and skills through continuing education and professional development.
  • Seek opportunities for involvement in practice-based research and engage in knowledge translation through evidence-based practice
  • Reviews referrals and prepares reports as required
  • Participates on appropriate committees pertaining to the operation of the unit/ service including quality improvement initiatives and participates in discipline-specific activities within Sunnybrook
  • In support of Sunnybrook’s full affiliation with the University of Toronto, participate in teaching Physiotherapy students on an ongoing basis including acting as a preceptor for fieldwork placements. Supervision of at least one student placements annually be expected

Qualifications/Skills:

  • MHSc. [P.T.] or equivalent
  • Registration with the College of Physiotherapists of Ontario
  • Membership with the Canadian Physiotherapy Association of Ontario preferred
  • Minimum 2 years’ experience working with older adults with frailty
  • Experience in Outpatient setting preferred
  • Working knowledge of community resources, OHAH, RGP, and specialized seniors programs
  • Training in Gentle Persuasive Approach (GPA), Dementia care an asset
  • Knowledge of Comprehensive Assessment an asset
  • Develop individual care plans that address functional mobility, home safety for the purpose of keeping seniors living at home and connecting them with community supports/resources
  • Experience facilitating group therapy and education for seniors
  • Experience in leading Falls Prevention Programs is preferred
  • Good working knowledge of mobility equipment/aides to optimize independence, and/or working with vendors an asset; ADP authorizer status preferred, or expected to gain ADP Authorizer status within the first year of working in this role
  • Well- developed skills in time management, problem solving, adaptability and collaboration in a team environment required
  • Proficient with virtual care platforms (Zoom, Teams, OTN, etc)
  • Strong ability to work in an interprofessional team environment, with well-developed communication and interpersonal skills to manage conflict and optimize client’s experiences
  • Ability to manage multiple tasks, set priorities, and be flexible in an ever-changing environment
  • Strong commitment to interprofessional collaboration, recognizing overlap in scopes of practices between health professions and able to work to fullest scope
  • Strong documentation skills and computer literacy required
  • Demonstrated commitment to person-centred care principles and best practices
  • Ability to demonstrate reflective practice.
  • Experience and commitment to quality improvement initiatives
  • Experience and rostered for pelvic physiotherapy and asset
