The renal transplant PAA is an integral member of the renal program team. The successful applicant will work closely with our Sunnybrook transplant coordinator, Data Manager, Renal Medical Director, Home Dialysis Medical Director and Renal Patient Care Manager. The PAA must be self-directed and work autonomously to complete tasks that require their input and or management. The renal transplant PAA will also be expected to work collaboratively with other transplant centres to help process and navigate our patients requiring transplant work up and support. In addition, the renal transplant PAA is also responsible for filling in as substitute during vacation, sick days etc for all outpatient clinics within the Nephrology Program, administrative services for patient care unit(s); and maintenance of patient records and statistics. The successful candidate must; Be comfortable and familiar with preparing and sending patient samples to different health care labs and destinations Organize and submit data in collaboration with the Data Manager to the Ontario Renal Network / Ontario Health Participate effectively and contribute to monthly program stats meetings. Manage Outpatient Clinics Coordinates testing schedule, in collaboration with the patient and provides the patient with complete listing of tests. Good understanding of Accuro Database preferred. Have excellent verbal and written communication skills. Work collaboratively within an interprofessional team setting. Understanding of the Sunnynet Data Portal is preferred
