Reporting to the Patient Care Manager, the Occupational Therapist (OT) in the outpatient Neuromuscular/ALS Clinic works within a multidisciplinary team to provide safe and effective care to the Neuromuscular/ALS patient population. The OT utilizes a person-centred approach in considering the occupational performance issues, component skills and environmental factors having an impact on the functional level of their patients. The OT will be a champion in promoting excellent patient care while also demonstrating exceptional leadership, communication, problem-solving and conflict resolution skills. The OT actively participates in interprofessional team activities such as team meetings and quality improvement initiatives. The OT supports the Philosophy of Person-centered Care; provides education/counseling, and documents information according to appropriate standards.   Qualifications/Skills Successful completion of a recognized post-secondary program in Occupational Therapy and minimum 2 years of experience Current registration in good standing with the College of Occupational Therapists of Ontario. Certified ADP Authorizer in seating and mobility (preferred) Previous experience and skills working with Neuromuscular/ALS patients and/or patients with complex needs preferred Experience or interest working with individuals at end-of-life preferred Excellent professional judgment, conflict resolution and interpersonal communication skills with patients, families, team members, students and community partners. Must possess strong written and oral communication, critical thinking, care coordination, and time management skills Competence in the practice of evidence-based occupational therapy and interprofessional care Experience in assisting with completion of CPP forms, insurance forms, referrals to various agencies/physicians will be an asset Ability to work effectively both autonomously and within an interprofessional team, ability to manage a diverse workload while setting priorities and meeting deadlines Familiarity Accuro will be an asset
