Summary of Duties:

Sunnybrook Health Sciences Centre has an opportunity for a Registered Dietitian to provide nutrition management to patients in the Peter Cipriano Centre for Seniors Health. The Cipriano Centre includes a Day Hospital (rehab), Outreach Team and Geriatric Medicine & Specialized Clinic with a vision to help seniors remain healthy and independent in the community.

 

 In accordance with Clinical Nutrition Standards of Practice as well as the Sunnybrook Health Sciences Centre person-centred care standards, the successful candidate will work across the Cipriano Centre allowing for seniors’ health needs to be addressed in a collaborative and comprehensive manner, such that care is integrated in partnership with community services to provide care as One Team across sectors.

 

Summary of Duties:

  • Assess, plan, implement and monitor nutrition care for patients in the Cipriano Centre in collaboration with the interprofessional team. 
  • Evaluate RD interventions using principles of person-centred care and evidence informed practice.
  • Act as a resource to patient/families and the interprofessional team in the Cipriano Centre
  • Maintain clinical documentation and workload measurement in accordance with organizational and professional standards;
  • Participate in the education and training of dietetic students;
  • Participate in professional activities, unit-based and organizational committees, quality initiatives, and practice-based research; 
  • Be a role model for exemplary senior friendly care and promote its senior friendly care principles in the Cipriano Centre;
  • Other duties as assigned related to departmental tasks.

 

Qualifications/Skills:

  • Member of College of Dietitians of Ontario (CDO)
  • Minimum 2 years of experience, with background in outpatient setting preferred
  • Previous experience in working with elderly populations preferred
  • Demonstrated person-centred care approach to care with an understanding of the nutritional  implications of nutrition problems affecting elderly clients
  • Experience in providing patient and family education, and clinical education for health care professionals
  • Ability to work effectively as a member of multiple interprofessional care teams
  • Demonstrated commitment to maintain and upgrade skills and knowledge through  professional development
  • Demonstrated initiative, and self-directed learning and practice
  • Proficient caseload prioritization and time management skills.
  • Excellent collaboration, communication and teamwork skills.
