Right now we’re looking for a Full-Time Registered Nurse to work in our Outpatient Mental Health Program. Working within a Stepped Care Model, this position supports our Centralized Intake Desk.

Reporting Relationship: Manager of Mental Health

 Hourly Rate Range: $39.07 - $56.00

Schedule Requirements: Day shift Monday to Friday, some evenings may be required

Location: Wilson Site - Must be willing to work on all Mental Health Units

Position Responsibilities:

 

  • Process and review external/internal mental health referrals directed to the Outpatient Mental Health program
  • Liaise with referral sources for clarification of referral and provide instructions regarding referral criteria to ensure complete and appropriate information is received
  • Conduct intake screener assessments (via telephone, video or in person) to clarify needs of patient and offer services best suited for the referral
  • Conduct risk assessments and interventions when clinically indicated
  • Provide patients with relevant mental health and addictions community resources
  • Deliver patient educational groups (via video or in person) for referred patients
  • Present complex referrals/assessments in team clinical rounds and communicate decisions with referral source and patient as appropriate
  • Participate in department related projects or committees to improve intake processes
  • Liaise with internal mental health team members to ensure correct placement of referrals
  • Provides coverage to other areas of the Outpatient Mental Health program as needed.

Qualifications:

  • Current registration with the College of Nurses of Ontario.
  • Current BCLS/CPR certification required.
  • BScN preferred.
  • Certification in Psychiatric Mental Health Nursing required, or willingness to obtain within one year of hire.
  • 2 years of recent mental health/addictions experience is required.
  • Demonstrated ability to provide individual/group health teaching for patients and staff.
  • Excellent assessment skills, including demonstrated knowledge of crisis intervention skills.
  • Demonstrates awareness and understanding in working with individuals from different cultures.
  • Demonstrated ability to work with complex patient and family situations
  • Demonstrates an excellent understanding re: the etiology of mental health/addiction illnesses.
  • Excellent knowledge of the Mental Health Act.
  • Knowledge/experience in Trauma Informed Care preferred.
  • Technology literacy and usage skills preferred.  Such as: video conferencing using MS Teams, OTN etc, and ability to use mobile technologies such as smartphones and tablets to connect patients to care.
  • Demonstrated knowledge of Mental Health & Addictions community resources is an asset
  • Demonstrated excellent communication skills, both written and verbal
  • Excellent interpersonal, organizational and problem solving skills, with an ability to set priorities and meet deadlines.
  • Must be able to work effectively, both individually and within a group setting, in a fast paced environment.
  • Demonstrates initiative and creativity in the workplace.
  • A positive attendance record.
