At Humber River Health we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve. In December 2017, the Central Local Health Integration Network (LHIN), in partnership with Humber River Health, North York General Hospital, Southlake Regional Health Centre, Mackenzie Health and Oak Valley Health, launched an integrated, innovative model of care delivery called the Reactivation Care Centre (RCC). The RCCs are located in the former Humber River Health Finch Campus and Church Campus respectively, both of which were extensively renovated for this new shared facility.
The Finch RCC, a first-of-its-kind collaboration brings four hospitals under one roof to provide specialized care for patients who no longer require acute care, but are waiting in hospital for transfer to an appropriate alternate discharge destination such as convalescent, slow stream rehabilitation, or long-term care. Each of the hospitals has its own unit which is staffed and operated as an extension of its hospital. Humber River Health operates one 32-bed patient care unit. Planning is currently underway for the opening of the 12-bed specialized Acute Behavioural Assessment and Management Unit (ABAMU) at the RCC Finch Campus.We currently have an exciting opportunity for a Clinical Manager, Reactivation Care Centre - Finch Unit and ABAMU to join our Post Acute and Reactivation Care Centres leadership team.

Responsibilities:
- Provides effective, efficient and safe patient services through human, financial, and environmental resource management.
- Supports a model of patient centered care that is based on best practice, program and corporate standards, and continuous quality improvement.
- Builds a professional practice climate and facilitates an interdisciplinary approach to patient care.
- Collaborates with internal and external stakeholders and community partners to plan, implement and monitor delivery of patient and family centered programs and services.
- Provides leadership in advancing the goals and objectives of the program.
- Coaches and mentors staff to navigate successfully through change.
- Manages all aspects of employee relations including recruitment, retention, attendance and performance management in collaboration with Human Resources and Labour Relations.
- Provide a safe environment for patients, visitors, staff and volunteers who work, visit and seek medical attention at RCC
Qualifications:
- Current registration and membership in good standing with the College of Nurses of Ontario (CNO).
- Membership with Nurses Association of Ontario (RNAO)
- Masters degree in Nursing, Health Administration or MBA required.
- Minimum of (5) years of clinical experience required, in an acute care hospital
- A minimum of three (3) years of management experience in acute care, medical/surgical services and/or geriatric care in a community based hospital
- Demonstrated recent knowledge of HRH/Acute and Post Acute Care and Home and Community Support Services priorities, policies, practices and service standards
- Demonstrates knowledge of Senior Friendly Framework and Alternative Level of Care Leading Practices
- Proven experience with Budget management, policy and process development
- Experience in effective critical thinking, decision making and problem solving skills within a fast paced, complex and continually changing environment.
- Experience in leading change, building high functioning teams and effective working relationships.
- Skills in strategic planning, project management, program development and effective resource management and utilization.
- Excellent attendance and discipline free record required.
