Job Description: Reporting to the Primary Health Care (PHC) Manager, the Primary Health Care (PHC) Nurse is an integral member of the PHC team, providing and coordinating initial, continuing, and comprehensive nursing services to patients in the PHC site. The PHC Nurse promotes quality health care outcomes working within established standards to ensure patients/clients receive a high standard of primary health care. The PHC Nurse works alongside physicians, clinic staff, and other health care professionals to promote quality health care outcomes, while working with established standards to ensure patients/clients receive a high standard of primary health care.PHC Nurse initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC Nurse works relatively independently and is expected to make decisions based on competent assessment knowledge and skills.

Experience

  • Experience in teaching adults, facilitation skills, and leading group activities
  • Minimum one year of recent nursing experience in a clinic-based setting required

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Basic Life Support Certificate (BLS)
  • Nursing diploma is acceptable

  • Knowledge in LEAN management system
  • Knowledge and application of quality improvement principles, tools, and outcomes
  • Ability to use health promotion and prevention strategies for program planning

  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

  • Experience in case management in health care setting is an asset
