Job Description: Reporting to the Chronic Disease Management Manager, Respiratory Programs, the Chronic Disease Management Nurse Educator is responsible for chronic disease management services related to respiratory conditions, including asthma. Key duties include assisting with the development, coordination, & delivery of inpatient and outpatient programs;assessing client self-management needs & planning appropriate interventions to ensure timely access to care; & providing individual education & counseling to patients, families, and caregivers on respiratory disease management & treatment. The role collaborates with families, physicians, dietitians, nurses, and community agencies, facilitates group education sessions in various settings, develops educational resources, delivers in-service training, participates in program development, quality improvement, evaluation, and research activities, & supports program and resource development through needs assessment, planning, implementation, & evaluation.

Experience

  • One (1) year experience in program/resource development
  • One (1) year experience in adult education
  • Three (3) years nursing experience with patients/families with chronic disease

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Basic Life Support Certificate (BLS)
  • Basic Life Support Instructor Certificate (BLS)
