Description
The Acute Care Program is seeking a casual Nurse Practitioner (0.25 FTE) to provide care within the Virtual Urgent Care Clinic and Bridging Service.
The Virtual Urgent Care Clinic provides timely, specialized mental health care through a virtual emergency department model. The Nurse Practitioner will work primarily within the Virtual Urgent Care Clinic and, when required, as part of the interprofessional team in the Bridging Service.
The Bridging Service is a drop-in, low-barrier, rapid-access service that provides short-term care to clients who have recently been discharged from an acute care inpatient unit or diverted or referred from the Emergency Department. Centrally located within the Acute Care Program, the service supports continuity of care through a range of clinical and psychosocial interventions, including medication management, housing and financial support, and psychosocial support.
Reporting to the Manager of the Bridging Service, the Nurse Practitioner works collaboratively within the interprofessional team and liaises with teams across other services. The Nurse Practitioner provides comprehensive, client-centred care while working autonomously and accountably within the scope of practice of a Registered Nurse (Extended Class) [RN(EC)].
This position is located at 1051 Queen Street West. Day and evening hours may be required.
