If you enjoy a role in providing services to clients in a community based setting then this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA). In collaboration with the Registered Nurse Case Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

This is an ON-SITE position. Diploma preparation or equivalent as assessed and determined by CLPNA is required. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation, current within the last 12 months, required. Completion of a post-basic Medication Administration Course for LPN’s. Handling, moving and lifting heavy or awkward equipment and/or individuals are required. Ability to perform all work-related duties in accordance with regulated legislature. Must have 1-2 years recent Home Care or Community experience.

The successful candidate will be a Licensed Practical Nurse with 2+ years’ experience, including at least 1 year in a community setting. Strong clinical skills are required, including wound, catheter, surgical drain, and CVAD care, as well as medication administration (injections). The ideal candidate is a collaborative team player with strong communication, organizational, and interpersonal skills. Strong case management and the ability to work in a congregate setting are essential
