- Supervises the work of the team according to planning guidelines and geographical boundaries.
- Works with team members and Management to ensure adequate staffing requirements are met.\u00A0
- Holds regular team meetings and huddles to keep team members apprised of Key Performance Indicators (KPI\u2019s) and information necessary for excellent service delivery and team effectiveness.\u00A0
- Utilizes appropriate communication skills to obtain information for the client and/or family and appropriate members of the service delivery team.\u00A0
- Provides complete, relevant assessments of the client/family and their environment utilizing established procedures.\u00A0
- In collaboration with the service delivery team, contributes to the delivery of the client\u2019s care plan.\u00A0
- Documents problems identified using a specified framework.\u00A0
- Selects and implements strategies or interventions from established care plans in consultation with client and/or caregiver and health care delivery team.\u00A0
- Describes, evaluates and documents client\u2019s responses to care utilizing established procedures.\u00A0
- Utilizes best practice guidelines at all times.\u00A0
- Utilizes adult learning principles and established training materials to teach and enhance skill levels of service providers including Client Care Coordinator as appropriate.\u00A0
- Provides hands on personal care as required.\u00A0 \u00A0
- Completes home visits with service providers for quality service and competency assessments and to ensure performance at optimal levels to meet client service requirements.\u00A0
- Receives and documents all employee, family and client complaints/issues related to service delivery and takes appropriate steps to ensure satisfactory resolution.\u00A0
- Meets with individual team members to build education and learning plans and provides input to annual PDP.\u00A0 \u00A0
- Acts as clinical resource to the service delivery team.\u00A0
- Demonstrates effective time management, planning and organization of day\u2019s work activities and of team members.\u00A0
- In collaboration with the Client Care Coordinator, assigns visits to team as appropriate, with attention to urgent care requests including sick calls, missed visits and extra visits.\u00A0
- Advocates on behalf of clients to ensure quality delivery of care is provided by utilizing established guidelines.\u00A0
- Promotes client-directed care, self-care and independence through teaching.\u00A0 \u00A0
- Works collaboratively with Directing Registered Nurses, Health Care Technicians (PSWs), Management and patients.\u00A0 \u00A0
- Registered Practical Nursing (RPN) diploma.\u00A0\u00A0\u00A0
- Minimum 5 years\u2019 experience in community based health care system.\u00A0\u00A0\u00A0
- Registered with College of Nurses in province of practice.\u00A0\u00A0\u00A0
- Team Lead or supervisory experienced preferred.\u00A0\u00A0\u00A0
- Proficient nursing skills with the ability to read and interpret technical and clinical instructions.\u00A0\u00A0\u00A0
- Clinical practice experience preferred.\u00A0\u00A0\u00A0
- Current CPR Certification.\u00A0\u00A0\u00A0
- Health teaching and facilitation experience preferred.\u00A0\u00A0\u00A0
- Strong observation and assessment skills.\u00A0\u00A0\u00A0
- Advanced computer skills required, including Microsoft Office Suite and Windows OS.\u00A0\u00A0\u00A0
- Strong verbal and written communication skills.\u00A0\u00A0\u00A0
- Interpersonal relationship building.\u00A0\u00A0\u00A0
- Ability to problem solve and adapt nursing/unregulated service provider interventions as appropriate.\u00A0\u00A0\u00A0
- Ability to work collaboratively as a member of an interdisciplinary team.\u00A0\u00A0\u00A0
- Experience in hands on personal care.\u00A0\u00A0\u00A0
- Coaching, mentoring and conflict resolution.\u00A0\u00A0\u00A0
- Must have personal vehicle and possess both a current driver\u2019s license and proof of vehicle
- Current criminal background check including a vulnerable sector screen.
- Must be able to wear Personal Protective Equipment (PPE)
