Location(s):\u00A0 This is a hybrid remote position. The position will work from home office and in the community 2-3 times per week to complete assessments. Travel to the Trenton office may be required as needed.Job Summary:
The Case Coordinator provides clients with comprehensive assessments ensuring a high-quality client experience. The Case Coordinator is responsible for developing, in consultation with the client and caregiver, a comprehensive care plan to meet the client\u2019s goals. The Case Coordinator supports the client and caregiver to manage their care budgets and required resources. Case Coordinators visit clients in their homes to complete case management tasks and are required to drive to client homes. Case Coordinators ensures navigation to appropriate resources and supports the philosophy of client self-management.\u00A0 Works within the framework of VON policies, procedures, standards, and quality & risk management.
Key Responsibilities:
\u2022 \u00A0 \u00A0Interpret and completes current standardized assessment tools such as the interRAI CHA, supports Intensity Scale, Application for Developmental Services, or other assessment tools as appropriate.
\u2022 \u00A0 \u00A0Completes client assessments using conversation based methodology.
\u2022 \u00A0 \u00A0Develops and conducts a comprehensive therapeutic rapport.
\u2022 \u00A0 \u00A0Identifies and prioritizes environmental risk for client\u2019s health and safety (shelter, food and running water, temperature control and overall condition of living environment).
\u2022 \u00A0 \u00A0Develops a comprehensive care plan (interpreting the Clinical Assessment Protocol and care planning).
\u2022 \u00A0 \u00A0Facilitates access to client identified services ensuring navigation to appropriate resources to support the care plan and goals.
\u2022 \u00A0 \u00A0Makes referrals to community and health resources as required.
\u2022 \u00A0 \u00A0With client consent, consults with partners in care service plans to ensure shared understanding of client needs and appropriate interventions/services to meet those needs.
\u2022 \u00A0 \u00A0Monitors client progress and adjusts care plans as required.
\u2022 \u00A0 \u00A0Reviews client care plan expenditures and supports clients to manage within the client care budget advocating for additional financial resources when needed for client care.
\u2022 \u00A0 \u00A0Accesses and reviews assessment.
\u2022 \u00A0 \u00A0Collects and reports relevant data to Manager on program outcomes as required by funder.
\u2022 \u00A0 \u00A0Adheres to the prescribed privacy policies and standards for Community Support Service or Developmental Services agencies including VON.
\u2022 \u00A0 \u00A0Positively and professionally represents all potential providers, external stakeholders, community agencies and service providers to clients while maintaining a depth of knowledge on the community resources.
\u2022 \u00A0 \u00A0Responsible to mediate and problem solve with multiple parties in a variety of contexts.

External and Internal Relationships:
\u2022 \u00A0 \u00A0Engages frequently with Administrative Support, Community Support and Home Care program Coordinators and Manager.
\u2022 \u00A0 \u00A0Networks and collaborates with all regional partners, Health Care and non-traditional providers and agencies.

Education, Designations and Experience:
\u2022 \u00A0 \u00A0University Degree or College Diploma in Health or Social Services field.
\u2022 \u00A0 \u00A0Minimum of 3 years related experience in a home and community care organization or social services program including experience in case management and experience in building external and community relationships.
\u2022 \u00A0 \u00A0Demonstrated experience in completing validated common assessment tools and maintaining competency.
\u2022 \u00A0 \u00A0Broad working knowledge of community resources, including service and programs within defined catchment area.
\u2022 \u00A0 \u00A0Knowledge and skills in comprehensive assessment and the ability to maintain proficiency.
\u2022 \u00A0 \u00A0Working with volunteers and seniors an asset.
\u00A0
Skill Requirements:
\u2022 \u00A0 \u00A0Strong attention to detail and accuracy.
\u2022 \u00A0 \u00A0Skilled in communication (verbal, written, listening), rapport building and relationship management.
\u2022 \u00A0 \u00A0Computer proficiency in Windows OS and Microsoft Office programs.
\u2022 \u00A0 \u00A0The ability to use analytical skills and understand/interpret client assessment data.
\u2022 \u00A0 \u00A0Ability to effectively manage multiple tasks, while prioritizing, meeting deadlines and working well under pressure.
\u2022 \u00A0 \u00A0Knowledge of social and developmental service sectors may be required
\u2022 \u00A0 \u00A0Demonstrated skills in leadership, team building.
\u2022 \u00A0 \u00A0Ability to educate client and families on available program resources.
\u2022 \u00A0 \u00A0Strong time management and organizational skills.
\u2022 \u00A0 \u00A0Ability to work independently and as part of a team.

Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
