Job Description:

Join a collegial and supportive team of family physicians at the Interprofessional Primary Care Team (IPCT) and Primary Care Teaching Clinic (PCTC) at Scarborough Health Network. We are seeking a family physician with an interest in medical education to support Primary Care delivery while contributing to the teaching and supervision of residents and medical students.

The successful candidate will be expected to obtain and maintain faculty appointment with the Department of Family and Community Medicine at the University of Toronto.

Responsibilities:

  • Practice in accordance with the College of Physicians and Surgeons of Ontario
  • Provide routine and preventative healthcare services to a patient panel of all ages with varying healthcare needs
  • Develop personalized treatment plans including medication management, lifestyle recommendations and referrals to specialists as required
  • Manage and monitor chronic conditions such as diabetes, hypertension and respiratory disease
  • Maintain accurate and up-to-date electronic health records to facilitate efficient and effective patient care.
  • Demonstrated experience in family medicine/primary care settings
  • Assess, plan, implement, and evaluate the needs of patients in collaboration with the interprofessional team
  • Perform patient assessments, medical history, physical, and psychosocial assessments.
  • Diagnose, treat, prescribe, order and interpret diagnostic imaging reports
  • Offer psycho-social assessments, service coordination, therapeutic and/or supportive counselling.
  • Implement appropriate health promotion, disease prevention and chronic disease self-management strategies for patients and families
  • Refer to various clinical resources, liaising between hospital, specialists, and community services
  • Ensure linkages and collaborative transitions of care to primary care, community outreach, and education support
  • Support evaluation and quality improvement activities to assess impact and outcomes and determine strengths, opportunities, and future directions of the IPCT services

Requirements:

  • Current registration and a member in good standing with the College of Physicians and Surgeons
  • Membership and Certification with the Canadian College of Family Practitioners (CCFP) is preferred
  • Strong knowledge and understanding of primary health care, chronic disease prevention and self-management, preferably in a community setting
  • 2-3 years recent clinical work experience as a Physician with community experience preferred.
  • Excellent verbal and written communication skills
  • Strong interpersonal, leadership, organizational and complex problem-solving skills
  • Ability to work well within an inter-professional team and to foster collaborative working relationships
  • Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
  • Utilize protective equipment provided and identify and reports hazards
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
