Description
* Baccalaureate Degree in a health related field or suitable combination of education and experience relevant to the position.
* Current or eligible registration with applicable professional licensing body.
* Minimum five years of professional experience in a relevant Health Care setting.
* Knowledge of critical incident investigative methodologies, prospective risk assessment methodologies and current patient safety initiatives.
* Proficiency with Microsoft Office programs.
* Knowledge of principles of adult education and learning strategies an asset.
* Demonstrated application of facilitation skills.
* Knowledge of applicable legislation and standards.
* Demonstrated leadership and supervisory ability.
* Demonstrated written and oral communication skills.
* Demonstrated experience in employee and labour relations.
* Province of Manitoba Class 5 Drivers License and access to a personal vehicle to provide service within the Prairie Mountain Health.
* Demonstrated experience in initiating change and improvement, including skills in workplace re-engineering within a Continuous Quality Improvement (CQI) environment.
* Ability to prioritize in a changing environment
* Sensitivity to the human and political dynamics of health care management.
* Strong organizational, decision making and problem solving skills.

POSITION SUMMARY:
The Patient Safety & Quality Improvement Coordinator plans, implements, coordinates and evaluates Patient Safety in Prairie Mountain Health. The Patient Safety & Quality Improvement Coordinator collaborates with internal partners to establish and implement processes to reduce preventable harm to patients, clients and residents in an effort to improve the quality of care and services.
The Patient Safety & Quality Improvement Coordinator focuses on promoting a culture of safety by conducting prospective and retrospective systems analysis, monitoring, sharing and learning from patient safety incidents, as well as providing patient safety and quality improvement education.
The Patient Safety & Quality Improvement Coordinator reports to the Manager of Quality Patient Safety and Accreditation and works collaboratively with staff, physicians, patients/residents/clients and their families and other external partners.
RESPONSIBILITIES:
Overview:
- Plan, implement, coordinate and evaluate a regional Patient Safety Program that encompasses the organization’s strategic direction, provincially and nationally endorsed patient safety initiatives and standards.
- Lead or participate in the systems analysis of incidents, critical incidents, critical occurrences, close calls, Mandatory reporting of Serious Adverse Drug Reactions/Medical Device Incidents that meet the review criteria for the purpose of improving system safety.
- Facilitate the development of action plans for systems analysis for the purpose of reducing the likelihood of recurrence.
- Monitor completion and follow up of developed action plans.
- Conduct or participate in proactive patient safety related risk in accordance with Accreditation Canada Required Organizational Practices.
- Provide critical occurrence reports and critical incident reports to Manitoba Health as per legislated requirements.
- Work with the management to facilitate disclosure of adverse events in keeping with policy and legislated requirements.
- Promote available supports to patients/residents/clients and/or family as well as staff involved in critical incidents.
- Utilize the Incident Reporting System (IRS) as an information source to monitor the type of patient safety events occurring for trending and identifying opportunities for improvement.
- Educate managers on the utilization and analysis of IRS data for the purpose of identifying trends and opportunities for improvement.
- Provide leadership and facilitation to implement regional patient safety initiatives related to current and future provincial and national best practice initiatives including those of the Canadian Patient Safety Institute, Manitoba Institute for Patient Safety and Accreditation Canada.
- Participate on regional and provincial patient safety and quality related committees, as required.
- Provide patient safety and/or quality improvement education for the purpose of promoting a culture of safety and improvement within the region to students, staff, physicians, patients, residents, clients, families and the public.
- Identify and assist with the development of patient safety and quality improvement related policies and procedures.
- Evaluate clinical health records to identify opportunities to improve patient safety.
- Analyze patient safety related reports and indicators with internal committees.
- Network with external patient safety related agencies/organizations.
- Maintain confidentiality of patient and employee information in accordance with policy and legislation.
- The focus of the responsibilities within this position description will shift depending on the organizations priorities and area of needed resources.
- The Patient Safety & Quality Improvement Coordinates will adhere to Provincial / Shared health goals as out lined in the Manitoba Quality Framework (2020)
