Position Summary
Penticton B.C. has exciting opportunity for a permanent full time Social Worker to support the Chronic Disease Management team at the Penticton Health Centre.

Salary Range:

Effective April 1, 2024, the pay scale for this position is $42.27 to $52.81

What Will You Work On?

The successful applicant will be working with a diverse demographic of clients to support prevention, self-management and whole person health working alongside the interdisciplinary teams of Pulmonary Function (Heart Health), Adult Diabetes and Pediatric Diabetes. This new role provides the successful applicant with an opportunity to support development of the Social Work role.

In this role, the Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Scheduling Information:

Shift types are days and shift types are from 08:00 to 16:30 hours.

Qualfications
Education, Training and Experience:

Bachelor’s Degree in Social Work from an accredited university.

Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.

Current full registration with the British Columbia College of Social Workers.

Current valid BC driver’s license.
