Position Summary
Creston Health Centre (CHC) is seeking a registered Social Worker to join our Home Health team. In this position, the Social Worker’s focus is providing coordinated, accessible, and client-centered care to individuals who require medical, personal, and social support in their homes. Days of work are rotating 08:00 to 16:00 and/or 16:00 to 22:00. A valid driver’s license is required. You may be required to drive a personal car on mileage and/or be assigned a fleet vehicle.

About the position:
The Home Health Transformation initiative is designed to support the growing demand for home-based care, driven by population growth and the increasing preference from clients to receive care at home. It aims to ensure that people can live independently with the support they need, in their own homes and communities. By incorporating digital health tools, telehealth, and other technologies, the initiative aims to improve communication and coordination among healthcare teams, as well as provide clients with easier access to care and support while in their homes.
The Social Worker will have a key role in a home health transformation team working both independently and collaboratively to provide exceptional social work services. Your responsibilities will include providing direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Qualfications
- Bachelor’s degree in social work from an accredited university.
- Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
- Current full registration with the British Columbia College of Social Workers.
- Current valid driver’s license.
