Position Summary
We are currently looking for a permanent full time Mental Health and Substance Use (MHSU) Housing Coordinator to join the Mental Health and Substance Use team at the Vernon Health Centre in beautiful and sunny Vernon, B.C.

What will you work on?


This position will be working closely with MHSU community teams to support housing referrals.

The Mental Health & Substance Use Housing Coordinator and practices within a patient/client and family centered model. This position works as part of an interdisciplinary team to support the coordination and placement of Mental Health & Substance Use clients into appropriate housing.

• Reviews referrals and conducts housing assessment to determine the appropriate housing model based on clients’ needs as directed by the MH clinician / physician, housing policies, and best practices.
• Refers clients based on clinical assessments performed by MH clinician/physician who require home support or adult day care participation to maintain independent community living.
• Collaborates, consults, and shares information for continuity of care and community resource coordination regarding mental health housing issues.
• Maintains appropriate client records, participates in in-services and team meetings, and engages in on going program planning and quality improvement activities.
• Ensures effective client access and flow within programs/services by incorporating standardization of practice and establishing relationships with interested and affected parties.
• Develops and implements mitigation strategies, as needed, to correct gaps in access and flow to programs/services.

Scheduling Information:

Shift types are days and shift times are from 08:30 to 16:30 hours.

Qualfications
Education, Training and Experience:

• Bachelor degree from an accredited educational institution in an Allied Health, Behavioural, or Social Science field relevant to the position,

• Two (2) year’s recent experience working with individuals with a mental illness, and experience working with housing programs or navigation of housing services or an equivalent combination of education and experience.

• Current valid B.C. driver’s license.
