POSITION: CARE ASSESSMENT SPECIALIST
Temporary Part-time

POSITION SUMMARY:
The Care Assessment Specialist is responsible for the coordination and overall support of the NRS/CCRS process. The Specialist provides leadership and direction to all staff and leaders involved in the completion and utilization of NRS/CCRS data and ensures all components meet current standards with respect to data quality and timelines. The Specialist is responsible for submission of complete and accurate data to the Continuing Care Reporting System (CCRS) and National Rehabilitation Reporting System (NRS) assessments and subsequent data corrections as indicated; support of leaders in the analysis of program-specific data with respect to clinical outcomes and quality improvement activities, and analysis data and trends to promote quality improvement activities.

This position is responsible for assessing the current state of operations, assisting to identify future state and establishing processes to support the quality processes related to CCRs and NRS data and utilization.

REPORTS TO: Director, Patient Care

DATE REQUIRED: As soon as possible

SALARY: $37.78 to $41.98 per hour

QUALIFICATIONS:
Education:
- Graduate of a recognized health care discipline or health information management program required
- NRS Assessor Certification required
- CHIMA Designation preferred

EXPERIENCE:
- Knowledge of medical terminology
- 3-5 years of experience in a healthcare environment
- Trained in completion of CCRS assessments
- Up to date knowledge of CCRS and NRS specifications as distributed by CIHI and with CCRS and NRS processes and data analysis

ABILITIES:
Knowledge/Skills/Abilities:
- Mature, flexible, organized and self-directed.
- Possess strong communication (both verbal and written), problem-solving and decision-making skills
- Ability to effectively and efficiently interact and collaborate with all levels of staff (e.g. Senior management, management, front line staff)
- Ability to exercise initiative in solving problems and identifying opportunities
- Experience with Meditech an asset
- Experience in information technology and its application in a clinical environment
- Demonstrated knowledge of assessment process, workload measurement as it pertains to CCRS and NRS and clinical information systems
- Knowledge and experience in CCRS and NRS reporting guidelines
- Ability to clearly and effectively communicate complex and detailed data into meaningful information.
- Knowledge of CIHI data sets, MOHLTC and access to reporting requirements
- Ability to manipulate and analyze data using various software tools (e.g. MS Excel, Access)
- Able to present information and express complex ideas clearly and effectively to leaders and front-line staff
- Team focused with a systems approach.
- Experience in clinical outcomes in relation to quality improvement initiatives
