Job Description: Under the direction of the Manager (or designate), and as part of the interdisciplinary Team, the Social Worker- MSW is responsible for the full scope of case management services for rehab patients adjusting to living with a disability. This is accomplished through collaboration with the patient, family, and other members of the interdisciplinary team. The Social Worker -MSW is responsible to develop a professional therapeutic working relationship with patients and families, including an understanding of the social work role, and SHA philosophy, objectives and policies. The Social Worker - MSW utilizes a range of interventional skills from a number of theoretical models and focuses on direct patient care   This position collaborates with acute care, primary health care, home care, mental health and addictions services, long-term care, and an array of government and non-government agencies to coordinate services to meet assessed needs.  The core functions of case management practice includes referral/intake, engagement, assessment, care planning, monitoring, evaluation, and termination/discharge. Case management is patient and family-centered, strength-based, flexible, cost-conscious, quality driven and includes advocating for and coordinating resources based on assessed need

Education

  • Master of Social Work (MSW)
  • Current registration with Saskatchewan Association of Social Workers (SASW).

Knowledge & Abilities

  • Ability to develop and maintain effective working relationships and communication with all contacts, including with social work colleagues and with multidisciplinary staff.
  • Knowledge of medical terminology and medical social work practices (e.g. assessment/documentation/discharge planning). 
  • Knowledgeable about the SHA, community agencies and resources to be able facilitate timely discharges. 
  • Intermediate computer skills. 
  • Possesses self-knowledge and understands own impact of self on work relationships and group process. 
  • Ability to work autonomously with high levels of responsibility and accountability. 
  • Ability to work independently and as a member of a multi-disciplinary team. 
  • Demonstrates strong communication and organizational skills. 
  • Demonstrates advanced assessment and intervention skills. 
  • Sound knowledge of master social work ethics and standards of practice is required, along with professional judgment and initiative. 
  • Ability to adapt to changing conditions with a high degree of flexibility.
  • Ability to work under stress and to prioritize needs of patients, team and health care system. 
  • Demonstrated ability to manage crisis situations in clinical areas relevant to the position.
  • Works within established policies and procedures, supporting patients to advocate for their individual needs. 
  • A high degree of accuracy is required in all aspects of responsibility, i.e.: professional judgements, documentation, effective written and verbal communication skills.
  • Willingness to adapt to new concepts, techniques and best practices in the academic and clinical fields of social work and health care
