Job Description:  Under the direction of the Manager (or designate), and as part of the interdisciplinary Team, the Social Worker - MSW - Senior is responsible for the full scope of case management services and counselling for Bridge Clinic patients adjusting to living with a chronic health condition. This is accomplished through collaboration with the patient, family, and other members of the interdisciplinary team. The Social Worker - MSW - Senior is responsible to develop a professional therapeutic working relationship with patients and families, including an understanding of the social work role, and SHA philosophy, objectives, and policies. The Social Worker - MSW - Senior utilizes a range of interventional skills from a number of theoretical models and focuses on direct patient care.

Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement

  • 2 years of relevant experience
  • Experience and training in the following: Solution-Focused Counseling 
  • Experience and training in the following: Cognitive Behavioral Therapy
  • Experience and training in the following: Acceptance and Commitment Therapy
  • Experience in a MH services environment providing psychological services 
  • Experience and training in the following: Motivational Interviewing
  • Experience and training in the following: Mindfulness-based Therapy

Education

  • Master of Social Work (MSW)
