Job Description: Reporting to the Manager or designate, the Social Worker provides a range of screening, triage, assessment, treatment, and case coordination services to primarily adult community mental health and addiction population. The Social Worker provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community.

Education

  • Bachelor of Social Work (BSW)

  • Knowledge in individual assessment and counselling skills
  • Committed to delivering on patient and family centered care
  • Commitment to self evaluation, professional development and ongoing self care
  • Ability to work cooperatively in an integrated interdisciplinary team
  • Knowledge in health promotion and prevention strategies
  • Ability to work with a variety of individuals presenting with complex issues
  • Systems Theory/Family Systems Theory, strength-based model, crisis intervention
  • Strong interpersonal skills
  • Expert knowledge and advanced skill in a variety of treatment modalities
  • Knowledge in local human service organizations, resources, referral procedures
  • Understanding social determinants of health for program development & delivery
  • Demonstrates strong communication and organizational skills
  • Ability to complete case management assessments, follow-up and treatment

  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • Valid Class 5 driver's license
  • One year experience in medical social work preferred
