Job Description: Under the direction of the Manager, Primary Health Care, this position oversees a range of Primary Health Care programs and a multidisciplinary team including professionals and paraprofessionals, ensuring smooth operation of those programs, providing supervision and direction for the staff, and consultation on complex health issues. The programs provide direct services in health promotion, community development, disease prevention, intervention, diagnosis and treatment for aggregates, groups, families and individuals in the community.
The main responsibilities are to: facilitate effective organizational planning and supervise the operating budget; supervision of human resources in an effective and positive manner; assess community trends within assigned area and modifies local services and assignments as required; ensure that the service delivered to clients is of a high and consistent quality; and support processes within the organization which lead to continuous quality improvement.
Primary Health Care programming is based on the circular model of holistic health care. Populations of each network will vary thus the service provision will reflect the uniqueness of each network based on their socio demographics, determinants of health and their demonstrated health care needs. The supervisor works collaboratively with internal and external partners and advisory committees to meet the needs of the community.

Experience
- Min three years’ experience in public health or community based nursing required
- One year exp/demonstrated ability to work with complex chronic pain patients

- Knowledge in the principles of Population Health Promotion
- Knowledge of Primary Health principles and practices
- Knowledge in adult education principles, methods and tools
