Job Description: The Primary Health Care (PHC) Registered Nurse (RN) is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The PHC RN promotes quality health care outcomes working within established standards for case management practice to ensure that patients/clients receive a high standard of primary health care. This will be done in accordance with Sunrise Health Region mission statement and Integrated Primary Health Services policies and procedures of the facility and department.
The PHC RN initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. The PHC RN reports to their respective Primary Health Care manager.

Experience
- Experience in case management is an asset
- Experience as RN (NP) in a Primary Health Care setting preferred.
- Experience in teaching adults, facilitation skills, and leading group activities
- Experience w/ clinical interventions in Integrated Care or Community Health Care
- 2 years of relevant experience
- Two (2) years clinical experience
- Minimum Two years (2) chronic disease management
Education:
- Bachelor of Science in Nursing (BScN)
Licenses:
- Valid Class 5 driver's license
Other Education and Training
- Master's degree preferred
- Master’s degree or equivalent combination of education and experience

Knowledge & Abilities:
- Knowledge in chronic disease prevention and management
- Demonstrated clinical competency

- Basic computer skills including Microsoft Office programs (Word/Excel/Outlook)
