Job Description: An integral member of the Primary Health Care Team and works in part of the network team in the medical home model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for behavioral health supports for a number of conditions under the care of the Primary Health Care Provider. The services to be delivered through this position include screening functional assessment and develops behavioral change plans in conjunction with the Primary Health Care Provider.  The Mental Health Therapist - Staff develops and implements a behavioral healthcare plan and makes referrals to as required

Education

  • Bachelor’s degree in a Human Services field

Licenses

  • Eligible for registration with applicable professional association in the Province of Saskatchewan
  • Valid Saskatchewan Driver’s License

  • Ability to lead group activities.
