Job Description: The Primary Health Counsellor reports to the Primary Health Care Manager. Professional peer review and responsibility will be maintained through a joint accountability to Mental Health/Addictions. The incumbent is a member of the primary health care team, in a collaborative, co-location model. The incumbent supports the providers in PHC services in screening, brief intervention and accessing appropriate services for ongoing mental health and psychosocial services for their clients. The incumbent screens and assesses the needs of referred clients and determines a treatment plan which may include short term intervention with the counselor and/or case management or referral to other community programs. The incumbent initiates/participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. He/she promotes quality health care outcomes working within established standards of practice to ensure that clients receive a high standard of primary health care.
