Job Description: The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to personal care homes and long term care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education
- Bachelor of Social Work (BSW)
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Bachelor of Social Work is acceptable
- Bachelor of Nursing is acceptable
Knowledge and Abilities
- Knowledge in the SHA, community resources and applicable programming
Other Information
- Previous Client Assessment Experience is an Asset
- Valid driver's license, where required
- Travel required
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
