Job Description: Provides case management services, which includes assessing, planning, monitoring and evaluating for determining the care needs of clients, collaborative coordination of services and case management process. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education
- Bachelor Degree in a related Health Science or Social Science discipline

- Registered/eligible registration with appropriate licensing body if applicable

- Ability to carry out duties in a manner that meets clients’ safety needs
- Negotiation, conflict resolution and mediation skills.
- Ability to communicate effectively within the virtual team
- Ability to manage a complex caseload with multiple demands
- Ability to sort emails to appropriate areas within shared email group
- Knowledge of community resources
- Work with individuals' family, caregivers, and community organization supports
- Demonstrates critical thinking, problem solving, conflict resolution skills
- Basic computer skills including Microsoft Office programs (Word/Excel/Outlook)
- Knowledge in time management
- Ability to establish and maintain interpersonal/consultative relationships
