Job Description: Provides assessment, care planning, client advocacy, and coordinating services and monitoring care needs. Participates in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community. Collects, distributes and documents client information, facilitating client conferences and assisting with active treatment phase. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"
Human Resources Exemption: No
- Min. 2 years of experience in a Mental Health and Addiction setting is required.
Licenses
- Valid Class 5 driver's license
- Degree in a health or social science related field.
- Communication, writing, presentation and interpersonal skills.
- Understands Mental Health Services Act, forms, policies & procedures of SHA/MHAS
- Ability to work under pressure and establish priorities
- Ability to work independently and as a member of a multi-disciplinary team
- Ability to plan, organize/coordinate efforts with other team members
- Knowledge of how mental health impacts the individual, community and families
- Ability to clearly articulate assessment findings
- Demonstrated knowledge in the area of adult assessment and intervention
- Ability to teach & train a variety of users in workshop and individual sessions
- Knowledge of local human service organizations, resources & referral procedures
- Ability to work independently and in collaboration with others as required
- Initiative in organizing work in accordance with departmental procedures
- Knowledge of human growth, development and human behavior.
- Knowledge in mental health/mental illness impact on client, family and community
- Make decisions in accordance with the SHA MHAS policies and procedures
- Registered/eligible for registration with the appropriate licensing body

- Experience with concurrent disorders and homelessness preferred.
