Job Description: Provides assessment, care planning, client advocacy, and coordinating services and monitoring care needs. Participates in policy and program development, health education, and promotion and liaison with other health professionals/service sectors in the community. Collects, distributes and documents client information, facilitating client conferences and assisting with active treatment phase.

Experience
- Two (2) years clinical experience
Education
- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Undergraduate degree in Health or Social Sciences
- Bachelor of Nursing is acceptable
- Bachelor of Physical Therapy is acceptable
- Bachelor of Occupational Therapy is acceptable
